subject to studio review, third-party platform availability, posted policies, and applicable federal,
New York State, and New York City consumer protections.
You confirm that the information you submit is accurate to the best of your knowledge, but 5th Ave Music
Studio does not guarantee that submitted information, scheduling outcomes, third-party systems, or
response times will be complete, error-free, uninterrupted, or available at all times.
By continuing, you authorize the studio to contact you by email, phone, or text about your request,
scheduling, billing, and follow-up service updates. Your online checkbox may be used as an electronic
acknowledgment of these request terms.
This is a practical request acknowledgment, not a substitute for a full legal services agreement. See
